SwissConsultationProtocolInput

Confirmations recorded in the _Erklärungen und Einwilligungen_ section for Swiss products.

Each field is one declaration printed on the application form, and sending it asserts the customer gave you that confirmation — so submit a protocol only once you hold all of them.

Every field is OnlyTrueValue and mandatory: the insurance system will not accept an application on which one is declined, so there is no way to express a partial protocol. If any confirmation is missing, omit consultation entirely and the customer completes the whole section on paper.

The signatories declare they had the opportunity to read the data protection information sheet and the section on data processing in the client information.

The insured person — and, for minors and wards, their legal representative — consents to the contract being concluded on their life and confirms the health questions were answered truthfully and in full.
